How the Rule Works
General Rule
As a covered entity under HIPAA, Maplewood Treatment Solutions must give you adequate notice of:
- How we may use and disclose your protected health information,
- What your rights are with respect to your information, and
- What our legal obligations are regarding that information.

1. How We May Use and Disclose Protected Health Information
We may use and disclose your health information only as permitted by HIPAA and applicable state law. Examples include:
- Treatment: We may share PHI with other healthcare professionals to coordinate your care.
Example: If you are hospitalized, we may disclose information necessary for your continued treatment. - Payment: We may use and disclose PHI to bill and collect payment for services provided.
- Health Care Operations: We may use PHI to assess quality of care, train staff, conduct audits, or manage operations.
- As Required by Law: We may disclose PHI to comply with federal, state, or local laws, such as public health reporting or subpoenas.
- To Prevent Serious Threats: We may disclose PHI to prevent a serious threat to your health or safety, or the health or safety of others.
- Business Associates: We may share PHI with outside entities that assist us with functions such as billing, data storage, or compliance, under HIPAA-compliant Business Associate Agreements.
All other uses and disclosures will require your written authorization, which you may revoke at any time.
2. Your Rights Regarding Protected Health Information
You have the following rights under HIPAA:
- Right to Access: You may inspect or obtain a copy of your medical and billing records.
- Right to Amend: You may request a correction of your PHI if you believe it is inaccurate or incomplete.
- Right to an Accounting of Disclosures: You may request a list of certain disclosures we have made of your PHI.
- Right to Request Restrictions: You may request that we restrict certain uses or disclosures; however, we are not required to agree to all requests.
- Right to Confidential Communications: You may request to receive communications about your health in a particular manner or location.
- Right to a Paper or Electronic Copy: You may request this notice in paper or digital form at any time.
- Right to File a Complaint: You may file a complaint if you believe your privacy rights have been violated.

3. Our Legal Duties
We are required by law to:
- Maintain the privacy and security of your protected health information;
- Provide you with this Notice of Privacy Practices;
- Comply with the terms of this notice; and
- Notify you if a breach occurs involving your unsecured PHI.
We reserve the right to change our privacy practices at any time. Any changes will apply to all PHI we maintain, and the updated notice will be posted prominently in our facility and on our website.
Providing the Notice
We make this notice available in the following ways:
- At your first service appointment and upon request thereafter;
- On our website for public viewing and download;
- In printed form at our office for you to take;
- Electronically, upon your consent to receive it by email.
If services are provided electronically (such as telehealth or online intake), we will provide an electronic version of this notice and make reasonable efforts to obtain acknowledgment of receipt.
Organizational Options
Maplewood Treatment Solutions may operate as part of an organized health care arrangement with affiliated entities for coordinated treatment and operations. Each entity adheres to HIPAA privacy standards. A joint notice may be issued if shared services or programs are utilized, in compliance with 45 CFR § 164.520(d).
